You can register on our web portal to review claim status and copy of explanation of payment, or you can call 800-934-6302.
The W-9 form is required by the IRS, and we require a completed form before processing your claim reimbursement. A W-9 form can be found at https://www.irs.gov/uac/about-form-w9. The form gives specific instructions on completion, and must be an exact match to your income tax return. Please complete and return the completed form to BMA via fax, mail or email to email@example.com as soon as possible to ensure prompt handling of your claim.
